Request Form 1095-C

Form 1095-C is a tax document that shows information about the health insurance coverage offered to you and, if applicable, whether you enrolled in that coverage.

Do You Need Form 1095-C to File Your Taxes?

No.  You do not need Form 1095-C to complete or file your federal tax return.  If you had health coverage through the company, the IRS already receives this information directly from us.  You can keep a copy of the form for your records, but it is not required when filing your taxes.

How to Request a Copy

Beginning with the 2024 tax year, the IRS allows employers to provide Form 1095-C only upon request rather than automatically mailing it to every employee.  If you would like a copy of your Form 1095-C, you may request one in any of the following ways:

Your Form 1095-C will be mailed or made available to you within 30 days of receiving your request.

Who May Request a Copy

All individuals covered under the company’s health plan — including current employees, former employees, spouses, and dependents — may request a copy.
